REGISTRATION INSTRUCTIONS

PLEASE READ THE REGISTRATION INSTRUCTIONS CAREFULLY AS OUR REGISTRATION PROCESS HAS CHANGED.


This year we have implemented a convenient new online registration system which allows you to set up a family account, register players, and make payment for our great program 24/7.  No need to wait in long lines on a limited number of registration days. Simply follow the instructions below, and ask a couple of friends to join. We can accept payment by credit and debit cards online. You will have the opportunity to register as a volunteer coach (Highly Needed), assistant coach (Much Needed), referee (Definitely Needed), or other positions. And as usual, to protect our children, all volunteers must submit to a background check performed by our national office.

OPTION  ONE STEP ONE


Click the Register Now button below to log into our new online registration system and Create a family based account. Set up a player profile for each child. Choose a program for each player. You will receive a confirmation email within minutes, so please ensure that your email address is correct. New Players need to bring proof of date of birth; and New Volunteers need to bring US government issued photo identification (as per the instructions in our registration system). Volunteers will need to bring this same ID to their mandatory parent orientation meeting.


OPTION TWO STEP ONE

2.) Print & Mail2copies of your online registration form
3.) Print & Mail1copy of your online volunteer form, e-signed.
4.) Copy of child's birth certificate or other proof of age {only if child has NOT played for Concord before}
5.) Mail the above along w/ a check to:

PLAYER FEES (includes a uniform, ball and medal)

* u06 / 4 & 5 year olds: $105 ($30.00 discount before May 10th)
* u08-u19 / All other ages: $130 ($30.00 discount before May 10th)
* VIPs: $105 ($30.00 discount before June 1st)


STEP THREE ALL REGISTRATION OPTIONS


Attend
 the mandatory Parent Orientation meeting where you will receive important information about our program including player(s) team assignment and parent(s) volunteer assignment. The meetings are usually scheduled at the beginning of the season and prior to start of practice.  VIP orientation will take place at the start of the season.

PLEASE REMEMBER TO CHECK YOUR EMAIL AND THIS WEBSITE FOR UPDATES ON THE PARENT ORIENTATION IN THE WEEKS AND MONTHS TO FOLLOW.

REFUND POLICY
Refund requests will be honored minus a $45 processing fee per player, before uniforms have been distributed.  Once uniforms are distributed, there will be NO REFUNDS granted.  To request a refund, send a self addressed stamped envelope to:
